First Appointment

Welcome to Santa Monica Fertility Specialists.

Please look over the following information. It outlines some of our policies and will also let you know what you need to bring with you to your first appointment.

Medical Information

Please be sure that you have your medical records from your referring physician. You can arrange to have them sent to you or you can pick them up from their office. We ask that you do not have them sent to our office, as we prefer that you hand-carry them when you come to your appointment.

Fees

We require that all office visits, procedures and medication charges be paid for in full at the time of service. We will provide you with an itemized statement for you to submit to your insurance company for reimbursement. The cost of your medical care will vary based on your diagnostic and treatment needs. Laboratory and x-ray charges will be billed separately by the providing facility.

Santa Monica Fertility Specialists works with several pharmacies that specialize in fertility medication and can offer competitive pricing. If your insurance plan covers fertility medication, these pharmacies can help you determine the details of your coverage by speaking with your insurance company directly.

Insurance

We will ask for your insurance information at your first visit. Please remember to bring your insurance ID card, group number, policy number, mailing address and phone number for accurate filing of claims. We would like to remind you, however, that while we require your insurance information, it is for emergency purposes only, as all procedures, medications and other related expenses are to be paid for at the time of service. We will provide you with the appropriate itemized statements for you to submit to your insurance company for reimbursement. While we cannot guarantee reimbursement for fertility care, we can advise you on billing procedures.

Travel Time and Arrival

Please allow thirty additional minutes driving time if you are traveling at peak traffic hours for your appointment. Your first visit will require at least an hour and fifteen minutes. Parking is available in the underground garage in our office building.

If for any reason you are unable to keep your appointment, please call us to cancel or reschedule for another date. We do require at least a 24-hour cancellation notice. Please contact our office if you require further information on our cancellation policy.
